What information do I need to report a claim?

In most situations, we only need the basics (what happened, when did it happen, where did it happen, who was involved)
The more information you can provide, the better but don’t wait to report your claim.

For Worker’s Compensation Claim, generally we also need the information about the injured employee (date of birth, address, social security number)
